Only one official BISP website provides the application form to apply for BISP registration. Here are the steps to download the forums and register your CNIC on BISE:

Please visit the official website https://www.bisp.gov.pk/ and click on the Download tab.

Step 2

Now click on Download Templates which will take you to the next tab.

Total 3 forums are available on BISP official website > Downloads > Download Templates. You have to download all 3 of these forums:

• Intruder Performa
• Power of Attorney
• Witness Performa

Fill BISP Forms

Now you have downloaded 3 different models to your device. These are the application forms and you have to fill in these forms with details. Therefore, here we describe a method for filling these forms in short form.

Intruder Performa

The first form, “Intruder Perform”, relates to the applicant’s personal information and details. On this form, you must provide your name, your father’s name, CNIC number, and city name. Furthermore, you have to provide the details of your family members in the sections on this form.

In addition, provide the applicant’s signature/thumb including the name of two witnesses + CNIC numbers. You also have to certify this form (from a government official).

Power of Attorney

The second form is the Power of Attorney and also includes the complete details of the applicant including signature and CNIC number. Further, in case of any false details, the government has the right to take action against the applicant.

Witness Performa

The last form is “Tasdeek Nama” which includes the two witnesses’ signatures and CNIC number. Accordingly, all these forms are filled out by the applicant just to confirm whether the applicant is needy or just a fraud.

Mail Forms

Finally, when you have downloaded and filled out these forms successfully, please mail them to Nadra’s official address. And then you can mail it to NADRA Headquarters, Islamabad Pakistan.

BISP Fund

You will receive 12,000 from the Benazir Income Support Program. However, this pleasure may increase because people do not receive it for several months and once they get the full money.
